A short course designed for clinicians and managers who are responsible for conducting incident investigations or who have an interest in better understanding the process from an adverse event, through investigation and analysis to recommendations and changes implemented to prevent similar incidents from occurring in the future.
Course Aims
Objectives of the workshop is for participants to investigate and learn from incidents using systematic methods of investigation.
By the end of the course delegates will:
- Understand theory & process underpinning systematic investigation including Root Cause Analysis (RCA)Learn to identify causes of process failures within their clinical teams
- Understand the investigation and analysis
- Gain experience of the practical application of some of the tools available to the incident investigator
Course Method
The course consists of a mix of lectures and hands-on practical sessions on various aspects of the incident investigation and analysis process. The delegates will have the opportunity to work with a case study where they will have the opportunity to use some of the systematic incident investigation tools and techniques
Course Content
The course will start with an overview of the theory underpinning systematic incident investigation and analysis. The 5 Stages of Incident Investigation will be discussed in detail with groupwork for participants to gain practical experience of key stages of Incident investigation using a case study:
- Getting Started
- Gathering & Mapping
- Problem Identification
- Analysis of Problems
- Solution Development
